Patellar Tendinopathy

Pain at the tendon just below the kneecap, typically load-related.

Also called: Jumper's knee.

This tendon tolerates high load in jumping and change-of-direction sport. Problems usually follow an increase in that load — more volume, more intensity, or a return to sport after time off.

It's typically localized to a specific point just below the kneecap, distinguishing it from more diffuse knee pain.

Why this matters

It tends to warm up with activity and hurt more afterward, which makes it easy to keep pushing through and delay addressing it properly.

Managing training load appropriately, rather than complete rest, is usually the more effective long-term approach.

Landing mechanics depend on coordination and timing between multiple muscle groups, not just strength, which is why movement quality is assessed alongside tendon load.

Fatigue late in training or competition can degrade landing mechanics, which is part of why this often appears or worsens as a season progresses.

  • Posture and movement
  • Balance and coordination testing
  • Eye movement or neurological screening when appropriate
  • Grip or motor-function testing when appropriate
  • Palpation and spinal function
  • Other examination procedures based on the individual

Common questions

Will I be adjusted on the first visit?
Generally no. The first visit is an evaluation visit. Dr. Geran reviews the findings and makes recommendations at the follow-up visit.
Do I need imaging before I come in?
Not usually. If your history or examination suggests imaging is needed, we will say so and help arrange the right next step.
